Healthcare Occupational health checks
The occupational health checks you'll need before registering on a healthcare degree course
If you're coming to uni to study a course in healthcare, you'll need to complete confidential occupational health checks and a disclosure and barring service check.
Healthcare checks
When studying for a course in healthcare, you are required under Public Health England, to be vaccinated against certain diseases to protect yourself and patients.

As part of your application, you are required to complete an Occupational Health questionnaire and obtain an Immunisation report from your GP. In order to facilitate this, you will be contacted by the ºÚÁϳԹÏHospitals University Trust (PHUT) Occupational Health Department. You will be able to complete your Occupational Health questionnaire and upload your immunisation report via a link that will be sent by email.
Please ensure that this is provided between 1st July and 31st August for courses starting in September. We are unable to accept any documentation prior to 1 July.
Providing this information in sufficient time allows PHU Occupational Health to process your application and clear you for course and clinical placement.
Please note: Not providing this information within sufficient time may delay your ability to attend clinical placements.
New students to the UK must ensure they are registered with a GP surgery.
1. PHU Occupational Health Questionnaire
PHU Occupational Health provide a confidential NHS service, designed to protect your health and wellbeing on course and placement.
If you are enrolled on a healthcare worker course (excluding dental) you are required to complete the PHU NHS Trust online confidential occupational health questionnaire.
2. Immunisation Evidence
You need to arrange for an to be completed. This can be obtained via the following methods, please allow sufficient time to gather this information:
- Requesting via your current GP surgery – this can take 4-6 weeks
- On the NHS App (you may need to request this access via your GP surgery)
- Childhood evidence (red books)
- Previous or current employers if immunisations have been given in employment.
Please Note, this must be stamped by your GP surgery or employer to be valid.

We do not recommend posting these documents.
If you are unable to obtain evidence of previous vaccinations you will be required to either have a blood test for immunity, and/or receive vaccinations as per the requirement for your course.

Disclosure and barring check
You'll do a criminal record (disclosure and barring) check during your induction week. To do this check, you'll be required to bring some personal documents. Visit the Government's website for .
You'll also need to provide details of your address history over the past 5 years including the full addresses, postcodes and the dates you lived at each address.
Overseas applicants are required to provide a Certificate of Good Conduct from your country. If you've spent 12 or more consecutive months in a single foreign country in the past 5 years, you'll also need to provide a Certificate of Good Conduct (with a certified English translation if necessary). The Certificate of Good Conduct should be dated within 6 months from your course start date. offers further guidance for many countries on how to obtain an overseas Certificate of Good Conduct.
Dental health checks
If you've been accepted onto a dental care course, you need to complete our dental health checks.
